General Evaluation Procedures: Every applicant seeking admission to any of MSD's educational programs must visit MSD with a parent or legal guardian for a complete evaluation before any decision can be made. Applicants 18 years of age or older may apply independently. The evaluation may include a personal interview, review of case history, and audiological, educational, and psychological testing. An assessment may be waived, if a comparable assessment, in the opinion of the IEP Team, has been completed during the previous six months and both the parents and the IEP Team agree to the waiver. The results of these assessments assist the IEP team in determining each applicant's educationally handicapping conditions, as well as what the most appropriate educational program and placement would be, including admission to MSD.
Prior to proceeding with the evaluation, the students' parent or legal guardian (or the student if they are over 18 years of age) will be provided with a copy of the Procedural Safeguards Booklet which explains the parents rights, confidentiality and what to do if there is a difference of opinion; and a copy of the MSD Parent/Student Handbook. The parent, guardian or the student (over 18 years of age) will also be asked to sign a consent for assessment form which will allow us to proceed with the evaluation. At the conclusion of the assessments, the parent, guardian or the student (over 18 years of age) will be told what the results of the assessments were as well as recommendations for educational placement.
It is also important to note that MSD requires any existing educational, psychological and medical files be provided to the IEP team prior to the evaluation date. These evaluations/reports will be used in conjunction with MSD's own evaluations in making a determination regarding appropriate educational placement.
Upon completion of the evaluation, the IEP team will share the results of all assessments with the MSD Evaluation Review Team (ERT). The task of the ERT is to determine the most appropriate placement for the applicant including whether or not the applicant meets the criteria for admission to MSD. If after the initial assessment at MSD, an applicant is determined to have additional handicapping conditions and/or the MSD Evaluation Team is unable to complete all necessary testing, further assessment may be required before an admission decision is reached.
