Application Process
There are three types of documents that you need to provide:
- Copy of your child's Birth Certificate.
- At least two (preferably more) documents as proof of residency in Maryland.
- If applicable, copies of the petition AND court order for legal guardianship.
Complete and return the following forms:

Authorization to Release Student Information Form
Please complete a separate release form for the current or most recent educational placement, as well as any outside therapy or medical service providers that includes their address and contact information.
